Book directly on our platform

Notes submitted to the Attendance Office must include following:

• Full name and date of birth
• Email address and phone number
• Preferred method of contact (call, text, email, portal)
• Reason for your visit or concern
• Brief description of symptoms or questions (if applicable)
• Wellness goals or preventive care requests
• Relevant medical history (optional but helpful)
• Current medications and allergies (if applicable)
